Cool baths and lots of fluids have been the way to handle it for us. We also do the alternating ibuprofen and acetominophen routine if it's a really high fever. For low fever, we just ride it out unless they have other reasons to need medication. But twice, it got so bad that we had to go to the hospital for IV fluids. My oldest was 2 and hit 105F after tylenol, and my youngest at about 18mo stayed right at 104 after ibuprofen AND tylenol and both were showing signs of heat exhaustion, so off we went.
